I, Zainab Musa, Female, businesswoman, Nigerian Citizen residing at no 12, Niyiola Street, Gwarinpa, Abuja, do hereby make oath and state as follows:

• I am the plaintiff/applicant in this case and by virtue of which I am conversant with the facts of this case
• I supplied to the defendant company steel raw material worth the sum of
• 2,000,000.00 (Two Million Naira)
• The defendant after receiving delivery of the goods promised to pay back with two weeks of supply.
• After over four weeks from the date of supply, the defendant as refused to pay the said amount of money owed after several demands.
• I know that the defendant has no defence to the claim
• The copies of vouchers and receipts issued to the defendant acknowledging supply of the goods is hereby attached and marked exhibits "A" and "B" respectively.
• I swear to this affidavit solemnly and conscientiously believing same to be true and correct and in accordance with the Oaths Act.
