REQUEST FOR MEDICAL REPORT OF DAYO ATU

We, Solicitors and Advocates to Dayo Atu (Our Client) have been duly consulted about his admission into the University Teaching Hospital, Psychiatric Department with Card No. 167980. His first appointment was on the 16th of April 2011 and the last appointment was on the 5th of May 2011.

We hereby request for his medical report/history which is needed to be able to defend him in court for an alleged criminal offence made against him while returning to Abuja by flight on a fateful day. Please find attached a copy his Patient's Reference Personal Card. His case, which is instituted in Chief Magistrate Court, Wuse Zone II, Abuja, is adjourned till 19th April 2012.
